I've felt it too. When my long relationship ended, I kept pressing my hand to my chest like I could hold something together. I told myself it was all in my head. It hurt anyway, all night. So I went and read the science, because that's how I cope. Here's what I found, in plain terms.

Your brain treats rejection a lot like pain

Scientists put people in brain scanners and had them look at a photo of the ex who just dumped them. The parts of the brain that switched on were some of the same parts that switch on when your body is physically hurt. So when you say a breakup "hurts," you're not just using a word. Your brain partly agrees.

There's even an odd finding: people who took plain Tylenol every day for three weeks felt a little less emotional pain than people who took a sugar pill. (Don't go take Tylenol for a breakup. The scientists weren't saying you should. They were showing how deep the link goes.)

Why are we built like this?

A long time ago, being alone was dangerous. If you got cut off from your group, you could die. So our brains learned to make the loss of people hurt, the same way a burn hurts, to make us pay attention and stay close to others. The ache in your chest is an old, caring alarm. It means you're someone who was built to bond with people. That's a good thing, even when it feels terrible.

Why you can't stop thinking about them

You might notice you keep checking your phone, keep wanting them, keep almost texting at 2am. That's not weakness. Scientists found that a recent breakup lights up the same brain areas as a craving for a drug. People in one study thought about their ex about 85% of the time they were awake.

So you're going through something like withdrawal. Your brain got used to having them around and hasn't caught up to them being gone. The craving comes in waves. Each time you ride one out without texting, your brain slowly learns the new normal. Be kind to yourself when it's hard. It's supposed to be hard.

One honest catch

You'll see headlines saying a breakup is "exactly the same as physical pain." That goes too far. Newer, more careful studies found that pain and heartbreak use brain areas that sit close together but aren't truly identical. Same neighborhood, different houses. What stays true is this: heartbreak is a real, body-level thing that overlaps with how your brain handles pain. That's solid. That's enough.

What helps tonight

Treat this like a real loss, because it is one. People often expect you to "get over" a breakup fast. The science says that's unfair. Give yourself room.

When you want to reach out to them, remember it's a craving, and cravings pass. You don't have to act on it.

Lean on other people. Being around friends and family who love you actually calms your body down. It's part of how you heal, so reaching out is doing something, even when it feels like nothing.

I went and pulled the research. What I found surprised me, comforted me, and in one important way turned out to be more complicated than the headlines say. I want to walk you through all of it, gently.

Your brain processes rejection in some of the same places it processes pain

Here’s the finding that launched a thousand articles. In 2003, a team led by Naomi Eisenberger ran an experiment where people lay in a brain scanner playing a simple online ball-tossing game with two other “players.” Partway through, the other players stopped throwing them the ball, a small, almost silly form of social exclusion. The brain treated it as anything but silly. When people were left out, a region called the dorsal anterior cingulate cortex lit up, one of the areas that registers the distressing part of physical pain, the part that makes pain feel bad rather than just informative. The more rejected people said they felt, the more that region activated.

Eight years later, Ethan Kross and his colleagues pushed the question closer to home. They recruited people who’d gone through an unwanted breakup in the previous six months, people who, by their own account, still felt intensely rejected. In the scanner, they looked at a photo of their ex and thought about being left. Then, in a different part of the study, a device delivered a genuinely hot (painful, though tolerable) sensation to their forearm. The regions that lit up while they looked at their ex overlapped with the regions that lit up during the physical burn.

When researchers say heartbreak “hurts,” then, they mean something more than a figure of speech. As far as parts of your brain are concerned, the loss of someone you love and a burn on your skin share some of the same machinery. There’s even a strange, telling footnote: in one study, people who took acetaminophen, ordinary Tylenol, daily for three weeks reported fewer hurt feelings than people taking a placebo, and showed less activity in those pain-and-distress regions. (Please don’t read that as a prescription. The researchers were emphatically warning against treating a breakup with painkillers; they were making a point about how deep the overlap goes. I’ll come back to why reaching for the off-switch isn’t the goal.)

Why on earth would we be built this way?

This is the part I find oddly moving. The leading explanation is evolutionary. For most of human history, being cut off from your people meant real danger rather than a bruised ego. A human alone on the savanna was a human at risk. So the theory goes that rather than build a whole separate alarm system for social loss, evolution did something economical: it routed the pain of separation through the alarm system we already had for physical injury. Tetherless meant threatened, and threatened needed to hurt enough to make you act.

Which means the ache in your chest right now is, in a sense, an ancient and deeply caring signal. It’s your nervous system treating connection as something you need to live. Feeling this is the cost of being a creature built to bond.

There’s also a withdrawal happening, and it explains the obsessive part

If the pain were the whole story, that would almost be simpler. You may have noticed something else going on, though: the constant thinking about them, the checking, the wanting, the 2am urge to send the message you know you shouldn’t. The way your mind keeps returning to them like a tongue to a sore tooth.

That has a different explanation, and it’s one of the most validating things I learned. Helen Fisher, Lucy Brown, Art Aron and their colleagues scanned the brains of people who’d recently been rejected and were still, in their words, “in love.” The scans showed activity in regions tied to craving and addiction, the same reward circuitry implicated in cravings for substances like cocaine and nicotine. In that 2010 study, people reported thinking about the person who left them for roughly 85% of their waking hours.

So when you feel like you’re in withdrawal, it’s because, neurologically, you sort of are. The reward system that learned to expect them, their texts, their voice, their presence, hasn’t gotten the memo that the supply is gone. It keeps firing, keeps expecting the fix, keeps you reaching. This is why “just stop thinking about them” is such useless advice, and why you should be tender with yourself every time you fail to follow it. The problem was never your willpower. You’re down-regulating a genuine attachment your brain built over months or years, and that takes time, because (as one researcher put it) every neural circuit devoted to that person has to be slowly updated to account for their absence.

Now the honest part: the science is more contested than most articles admit

Here’s where I have to be the librarian as well as the friend, because you deserve the real picture rather than a comforting oversimplification.

That tidy story, rejection literally uses the same circuits as physical pain, got enormously popular. It’s a beautiful, quotable idea. In 2014, though, a team that included some of the very same researchers (Choong-Wan Woo and Tor Wager among them) went back with a sharper analytical tool. Instead of asking “do the same broad regions light up?” they asked “are the actual patterns of activity the same?” And the answer was no. Using machine-learning analysis of the fine-grained brain data, they found that physical pain and social rejection produced distinct, separable signatures, even inside the regions everyone had pointed to, like the dorsal anterior cingulate. A classifier trained to recognize physical pain performed no better than chance at detecting rejection, and the same held the other way around.

Their conclusion, in plain terms: rejection does more than borrow the physical pain system. It involves its own representations that happen to sit in roughly the same neighborhoods of the brain. Same city block, different houses.

So which is true, is heartbreak physical pain or something else? The most honest answer I can give you is that both things are real, at different levels of resolution. Zoom out, and pain and heartbreak clearly share territory and overlap in real, measurable ways. That part has held up across many studies. Zoom way in, and they’re more distinct than the early excitement suggested. The headline “a breakup is literally the same as physical pain” goes too far. The careful version, that heartbreak is a real, body-level experience overlapping with how your brain handles pain, is well-supported, and it’s enough.

I’m telling you the messy version on purpose, because I think people in pain are usually stronger than the internet gives them credit for, and you don’t need me to flatten the truth to make you feel better. The honest truth is comforting enough: your suffering is real, proportionate, and a sign you’re doing exactly what a grieving person does.

What this means for you, tonight

A few things I’d gently offer, drawn from what the research actually supports.

The pain being real also means it deserves real care, the kind you’d give a genuine injury rather than something to tough out. We tend to grant grief to a divorce but expect someone to bounce back from a breakup by Monday. The studies don’t support that double standard; the distress of losing an unmarried relationship is, for a lot of people, genuinely severe. You’re allowed to treat this as the significant loss it is.

The withdrawal being real means the urge to contact them, to seek the “fix,” is a craving rather than a cue to act. It will spike and it will pass, the way cravings do. Every time you ride one out, you’re letting the reward circuit slowly learn the new reality. (There’s a whole separate piece to write about no contact and why it works. It has less to do with willpower and more to do with giving your nervous system room to recalibrate.)

And the part where love also acts as a painkiller, looking at a loved one has been shown to reduce physical pain in studies, cuts both ways here. It’s part of why the absence hurts so much; you’ve lost something that was genuinely regulating your system. It also points at the way out: the comfort of other people, the friends and family you can lean on, belongs to the healing itself. It’s closer to the actual mechanism than a distraction from it.

Heartbreak, more than one psychologist has noted, behaves a lot like grief, and a bit like a bad flu. There’s often no clever way around it, only through, and it fades into the background slowly rather than ending on a particular day. I don’t mean that as a grim sentence. The same body of research that explains why this hurts so much also consistently finds that it gets better. The attachment circuits quiet down. The cravings thin out. Where the idea came from: the anterior cingulate

The story starts in 2003, with Naomi Eisenberger, Matthew Lieberman and Kipling Williams. They used a deliberately trivial setup called Cyberball: you lie in an fMRI scanner believing you're playing a simple online ball-tossing game with two other people, and partway through, the other two stop throwing you the ball. That's the entire manipulation. It sounds like nothing. The brain disagreed.

Two regions stood out. The dorsal anterior cingulate cortex, which tracks the distressing quality of physical pain (the part that makes pain feel bad, as opposed to the part that tells you where it is), grew more active the worse people said they felt. The anterior insula, also tied to the felt unpleasantness of pain, came along with it. The correlation between self-reported social distress and dorsal anterior cingulate activity is the seed of the whole "social pain" idea. The reason this mattered is that it suggested rejection was being processed not by some separate emotional module, but by the affective machinery of physical pain.

It helps to know what the anterior cingulate actually does, because "pain region" is too narrow. It's better thought of as a conflict-and-alarm hub: it flags when something is wrong and worth your attention, whether that's a burn, an error you just made, or a social bond coming apart. So when people describe it as the brain's "this is bad, do something" detector, that's a fair gloss. Rejection setting it off is the brain classifying lost connection as a genuine problem.

From a lab game to a real broken heart

Cyberball is clean, but losing a stranger's ball-tosses is not losing the love of your life. So in 2011, Ethan Kross, Tor Wager and colleagues did the study that brought it home. They recruited 40 people who had been through an unwanted romantic breakup within the previous six months and who said they still felt strongly rejected. In the scanner, two things happened. In one condition, participants looked at a photograph of their ex and were asked to relive being rejected. In another, a thermal device delivered a hot, painful (though carefully tolerable) stimulus to the forearm.

The headline result: the ex-photo condition activated regions classically tied to physical sensation, the secondary somatosensory cortex and the dorsal posterior insula, not just the affective-distress regions from the Cyberball work. This is what the paper's title means by "shares somatosensory representations." Intense social rejection reached further into the sensory pain system than ordinary sadness or a Cyberball snub did. That's why the 2011 study, more than the 2003 one, is the one people cite for "heartbreak is like physical pain."

Worth holding in mind: this was a specific population, people in the acute, still-raw aftermath of an unwanted breakup. The finding is about that intensity. It doesn't claim every flicker of social awkwardness lights up your pain cortex.

Then there's the acetaminophen study, which I find genuinely strange. In 2010, Nathan DeWall and colleagues had people take either ordinary acetaminophen (Tylenol) or a placebo daily for three weeks. The acetaminophen group reported fewer hurt feelings day to day, and in a follow-up scanner session showed blunted activity in the dorsal anterior cingulate and anterior insula during social rejection. A common over-the-counter painkiller dialed down social pain. Please read that as a clue about shared machinery and absolutely not as advice. The researchers were not suggesting anyone medicate a breakup, and there are real risks to daily acetaminophen. The point is the overlap is deep enough that a physical-pain drug touched it at all.

Why a brain would be built to ache over people

This is the part I find moving rather than clinical. The leading account is evolutionary, and it's elegant. For nearly all of human history, a person severed from the group was a person in danger: less food, less protection, far worse odds. Social connection was as load-bearing as warmth or water. The argument, which Eisenberger develops in her later review work, is that evolution is thrifty. Rather than grow a brand-new alarm for social loss, it tethered social loss to the pain system that was already there and already good at commanding attention. Pain is, functionally, a signal that says something here threatens your survival, fix it now. Routing separation through that same signal made staying connected feel urgent in the body, not just sensible in the mind.

Which reframes the ache in your chest as an ancient and oddly caring piece of design. It's your nervous system treating the bond you lost as something you needed to live, because for your ancestors, that was literally true. Feeling this is the cost of being a creature built to bond, and it's the same wiring that made the love possible in the first place.

The other system: craving, not just hurting

Pain is only half the neuroscience, and on its own it doesn't explain the part that probably torments you most: the obsessive pull. The checking. The wanting. The mind returning to them like a tongue to a sore tooth. That has a separate explanation, and understanding it was the single most validating thing I read.

Helen Fisher, Lucy Brown, Arthur Aron and colleagues scanned people who had recently been rejected but were, by their own report, still intensely in love. What lit up was not mainly the pain system. It was the brain's reward and motivation circuitry: the ventral tegmental area, which makes dopamine, and regions of the striatum including the nucleus accumbens, the same broad system implicated in cravings for substances like cocaine and nicotine. Rejection didn't switch love off. In some ways it cranked the wanting up, the way a withheld reward makes an animal press the lever harder. In that 2010 study, participants reported thinking about the person who rejected them for roughly 85% of their waking hours.

This is why romantic rejection behaves so much like addiction withdrawal, because mechanistically it overlaps with it. Your reward system spent months or years learning to predict them: their name on your phone, their voice, the specific relief of their presence. That predictive wiring doesn't update the instant they leave. It keeps firing, keeps forecasting a reward that no longer arrives, and the gap between expectation and reality is experienced as craving. "Just stop thinking about them" fails for the same reason willpower alone fails against any craving. You're not fighting weakness. You're waiting for a learned prediction to extinguish, and extinction is a biological process with its own timeline. Every craving you ride out without acting is, quite literally, a trial in which the circuit gets a little evidence that the reward is gone.

Now the honest part: the tidy story doesn't fully survive scrutiny

Here's where I have to be the librarian as much as the friend, because the most-shared version of this science is cleaner than the science itself.

The popular claim hardened into "rejection uses the exact same circuits as physical pain." It's quotable, it's comforting, and it overreaches. In 2014, a team that notably included Choong-Wan Woo and Tor Wager (Wager having worked on the 2011 study, so this is in part the field auditing itself) re-examined the question with a sharper method. The earlier work asked whether the same broad regions activate. Woo and colleagues asked a harder question: are the actual fine-grained patterns of activity the same? Using multivariate pattern analysis, essentially training a machine-learning classifier to recognize the neural signature of physical pain, they tested whether that signature also detected social rejection.

It didn't. Within the very regions everyone had pointed to, including the dorsal anterior cingulate and insula, physical pain and social rejection produced distinct, separable patterns. A classifier tuned to physical pain performed at chance on rejection, and the reverse held too. Same neighborhoods, measurably different activity inside them. The image I keep returning to is: same city block, different houses.

So which is true? Both, at different resolutions, and being honest about the resolution is the whole point. Zoom out to the level of regions and felt experience, and pain and heartbreak genuinely share territory; that finding has held up and it's real. Zoom in to the level of fine-grained patterns, and they pull apart into separable processes that happen to be neighbors. The strong headline, "a breakup is literally identical to physical pain," goes too far. The careful claim, that heartbreak is a real, body-level experience that overlaps with how your brain handles pain while also being its own thing, is well-supported. And the careful claim is already enough to take your suffering completely seriously.

I'm walking you through the messy version on purpose. People in pain are usually far stronger than the internet assumes, and you don't need the truth flattened to be comforted by it. What survives every version of the science is this: your suffering is real, proportionate to a real loss, and a sign your brain is doing precisely what a bonded, grieving brain does.

What this means for you, tonight

Connection is, among other things, a biological painkiller. That cuts two ways here.
